Pull dma-mapping updates from Christoph Hellwig:
- optimize DMA sync calls when they are no-ops (Alexander Lobakin)
- fix swiotlb padding for untrusted devices (Michael Kelley)
- add documentation for swiotb (Michael Kelley)
